Healthwatch Stockport Social Care Task Group

During Engagement activity carried out with Healthwatch Members and members of the public in October 2022, it was agreed that Healthwatch would focus on Social Care as our new priority from in 2023.
Social care task Group

Primarily the focus will be on people who have been discharged from hospital and who have a social care need and are provided with a package of social care support.

There are 3 levels:

1.    People who are discharged/transferred back to their home or place of care [care home, supported living etc.]

2.    people who are transferred into a step-down facility for a period of intermediate or enablement care with the intention of going back home or back to their usual place of care

3.   People who are transferred to a new permanent facility due to a life changing event/illness in which they cannot return home to or to their usual place of residence

We will engage with people and their families/carers who have been or are going through this process to understand what their experiences have been or are as they move through the system.

We will work with and engage with commissioners, providers, staff, and other local organisations who support people around discharge to ensure we capture as full a picture as possible.

We will at:

  • Quality of care experienced when people leave hospital
  • Focus on inequalities  
  • Carer perspective
  • Where do people go to get information and advice about social care needs [secondary focus]
  • Social care assessment process – what are they do people understand them [secondary focus]
  • Quality of day care provision [commissioned focus]
  • Unmet need
